For patients with atrial fibrillation on DOAC, when should they resume DOAC after a thalamic bleed without intraventricular bleed?
How does presence of intraventricular hemorrhage change your recommendations?

I would recommend restarting a DOAC approximately 2 weeks after the intraventricular bleed. There is no good clinical trial to base this decision on so it is a clinical judgment.
