How would you approach a case of enteroblastic cholangiocarcinoma?  

It seems that this rare entity can be difficult to differentiate from a germ cell tumor. Is there anything characteristic about its histology, biology, or clinical features that can help? Molecular sequencing is sent. What would be your first line treatment?
