Should ITV be an expansion from GTV or CTV?  

The ICRU definition of ITV is that it is an expansion of the CTV, based on motion. However, in practice, many people expand the GTV to ITV (which is more appropriately called the iGTV) and then expand the ITV/iGTV to CTV. This gives you a larger target volume then the ICRU definition.
