What are your recommendations in managing pudendal neuralgia/red scrotum syndrome?   

I have 2 male patients who have failed oral gabapentin (sedation), tacrolimus and opzelura both burned, doxycycline x 2 months. They complain of burning and itching especially with heat and sweating, on the scrotum/penis.
