What is your approach to muscle-invasive bladder cancer in a patient who is ineligible for surgery and radiotherapy?   

Consider a patient who is too frail for surgery and ineligible for radiotherapy due to prior pelvic RT. Would you consider Pembrolizumab monotherapy vs EV + Pembro combination? TURBT?
