Practically, I find the following tool from Aukland very helpful.They mapped melanoma lymphoscintigraphy data onto an interactive tool that allows you to select different locations and estimate the nodal drainage.

I would electively treat the regional nodes if the risk is 10-15% or higher. Obtain a pretreatment CT to make sure that there are no clinically positive nodes. Obtain PET if Merkel cell carcinoma or melanoma.
