How do you approach an out of field intracranial recurrence for a GBM?
Do you approach it similar to a de novo GBM? Would you be more inclined to offer hypofractionated radiation given the poorer prognosis?

Any patient with recurrent GBM should have multidisciplinary evaluation for indivdualized clinical decision. For patients with out of field recurrence, I would still consider clinical trials as the first option.

We review at our multidisciplinary brain tumor board taking into account various factors including interval time to recurrence, patient's overall performance status and prior therapies. If the plan is to re-irradiate an out of field recurrence, generally my preference is for 35 Gy in 10 fx or even s...
