How does implant breast reconstruction/augmentation at the time of lumpectomy affect your adjuvant radiotherapy approach?
Do the same concerns as post-mastectomy radiation of implants apply?
There are some small case series' (https://www.ncbi.nlm.nih.gov/pubmed/21346535 DOI:10.1097/SAP.0b013e3181f8caf1 and https://www.ncbi.nlm.nih.gov/pubmed/27536475 DOI:10.1097/GOX.0000000000000800) of women who undergo reconstruction with implants after lumpectomy and radiation therapy, but data about women who undergo this procedure upfront, prior to radiation seems to be lacking.

We have done quite a few patients with adjuvant RT who had previous breast augmentation. I have used 45 Gy in 25 fractions followed by boost and make sure dose homogeneity is achieved and hot spots are lmited and small to decrease risk of contracture.
