I would treat with curative treatment for 7-10 days and then, if possible, treat with chronic suppressive low-dose oral therapy (depending on susceptibility data available) until the stone can be definitively removed/the source of infection removed.
After treatment of an ascending urinary tract infection for 7 days, the infection will be treated. However, if the stone is large enough it may be colonized and cause recurrent infection.
