Is there a role for external radiation and/or brachytherapy in stage IVB vaginal cancer aside from palliation?  

Disease involves the lungs diffusely and bilaterally.

If only palliative treatment is recommended, what should be the target volume (primary only? primary and nodes? cover inguinal LNs?)? What dose would you recommend? with or without concurrent chemo? Would vaginal brachytherapy provide any benefit after external radiation or just additional toxicity?
