Which factors would prompt empirical lead extraction in bacteremic patients without TEE evidence of lead vegetation?  

My point of view is to have a very low threshold to extract a device where infection is likely. The presence or absence of lead vegetation may be a bit misleading as lead thrombi are not uncommon.
