Are there any precautions you would take when radiating the pelvis in an obese patient with recent extensive ventral hernia repair to minimize dehiscence risk?

Post-operative endometrial with high-risk features for nodal recurrence, planning VMAT with >4-weeks recovery for complete closure of skin, with avoidance of abdominal mesh and anterior wall.
