Do you prefer IMRT to 3D for partial breast treatment?

Prefer IMRT for better conformity, dose homogeneity, and less dose to uninvolved breast. Lung dose is not a clinically meaningful difference.

Our practice for PBI is entirely IMRT/VMAT. Conformity can’t be beaten, and it is very fast to deliver co-planar arcs vs say, couch kick 3D. In the several years we have done 6 Gy x 5 VMAT PBI, we have seen nearly none of the breast issues seen with 3D BID PBI.We switch to mini-tangents a la U...
