Patient has bilateral common, internal, and external iliac and femoral DVT from obstruction by a large uterine mass that is likely malignant and is planned for resection.

This is a very difficult situation to manage.
Would promptly initiate therapeutic anticoagulation as long as no increased risks for bleeding.
