What do you recommend if you find persistent lymphadenopathy during CT simulation for adjuvant head/neck radiation therapy?  

Assuming the simulation was performed in a timely manner (within 3-5 weeks post op), in the majority of cases this would be considered an early recurrence despite an adequate neck dissection (min 15-20 nodes), and requires intensified salvage (rather than adjuvant) treatment.
