Yes, if a person with hypoparathyroidism has a low 25(OH) D level, on calcitriol, I would do several things; first, figure out why it is low; and second, check a serum calcium and phosphorus level. One explanation for low 25-HYDROXYVITAMIN D is calcitriol induces 24-hydoxlyase increasing its destruction. Yes, always had my patients on vitamin D to treat vitamin D deficiency and gave 50,000 IU of vitamin D2 or D3 once a week for 8 weeks then 50,000 IU every 2 weeks or their equivalent daily.
