How do you approach endocarditis prophylaxis for patients with rheumatic diseases undergoing invasive dental surgery and no history of joint replacement?  

Rheumatologists are repeatedly asked about giving antibiotics to patients having oral surgery such as tooth extraction for impaction.  This question is meant to address patient requests outside of the management of osteoporosis, focused on immunosuppressive use.
