If pre op ADIC failed and the RP LMS is completely resected with NED, I would just pursue close observation.

I would offer adjuvant chemotherapy for high risk patients... meaning those with high grade tumors which are larger than 5cm in diameter.
Such patients have a high risk of recurrence when treated with local therapies such as surgery alone and or XRT.
